Precision Brain Health Education Program — Annual Enrollment
Continue your brain-health education throughout the year.
You are enrolling in the Annual Precision Brain Health Education Program, an educational advisory program provided through the Pacific Neuroscience Institute Foundation and overseen by David A. Merrill, MD, PhD.
Your annual enrollment includes
Ongoing educational review and synthesis of relevant information
Educational interpretation of new results and reports as they become available
Longitudinal perspective on evolving brain-health priorities
Appropriate non-clinical educational communication outside scheduled visits
Educational coordination when multiple providers or programs are involved
Enrollment details
Program term: One year
Current program fee: $5,000 per year
Billing: Annual subscription with automatic renewal
Renewal price: The program price in effect at the time of renewal
Payment: Credit card through secure checkout
Renewal, cancellation, and refunds
Enrollment automatically renews annually to maintain continuity of educational support.
You will receive advance notice before renewal.
You may cancel your enrollment at any time.
Upon cancellation, a pro-rated refund for the unused portion of the current annual term may be issued in accordance with program policies.
Before enrolling
By completing enrollment, you acknowledge that:
The program provides educational services and does not change or replace your medical care or clinical relationships
Your existing clinical care remains separate and unchanged. If you currently receive medical care from Dr. Merrill or another clinician, you may continue that according to its existing arrangements.
The program fee does not include medical services such as diagnosis, prescribing, treatment, and ordering diagnostic tests. These services may continue to be provided separately by your treating clinician(s) when medically appropriate.
Enrollment does not provide or guarantee access to a treating physician beyond existing clinical-care arrangements
Clinical scheduling, appointments, and insurance-based services remain separate from this educational program
Appropriate non-clinical educational communication outside visits may occur through this program
Enrollment automatically renews annually unless canceled
Your enrollment establishes a year-long educational relationship designed to help you understand and integrate your evolving brain-health information. Enrollment does not create, replace, or change a clinical care relationship with Dr. Merrill or any other provider.
